From 5 Corners head east on Route 15/Main Street approximately ¼ mile. After crossing the train tracks, you will pass Church Street on the right. Our sign is located on the right side of the street between Apple Vision Center and Rich Funeral Home. Turn into our driveway and follow the road to the building in the back. We are happy to share space with Champlain Valley Foot and Ankle and Champlain Valley Obstetrics.
